After the tragic refinery explosion that killed five people and injured two in Anacortes, Washington, in April 2010, investigators from the U.S. Chemical Safety and Hazard Investigation Board arrived to survey the scene and the Washington Department of Labor and Industries began an investigation. A year earlier, inspectors for the agency had found 150 safety and health violations at the refinery, which was owned by San Antonio-based Tesoro Corp.
Perhaps ironically, just one month later, another refinery explosion took place in San Antonio, at a refinery owned by AGE Refinery Inc. That explosion was set off by an explosion of an 18-wheeler that was refueling at the refinery. This explosion critically injured one worker.
These two refinery explosions happening in two states one month apart served as a stark reminder of the hazards of refinery operations. Huge quantities of highly explosive fuels are concentrated in refineries, and a small fire or a localized explosion can promptly escalate and cause major property damage and loss of life in a matter of moments.
